Dr. Patricia Homer and staff, are excited to welcome
new patients to their fresh, compassionate and gentle
form of dentistry. Everything you need to know, prior to
your first visit, is on this page. Read through the
following directions carefully, and please let us know
if you have any questions.

Now, kindly download and fill out the following forms
before your scheduled appointment.



Make sure to include:
1. All medical issues, and allergies you may be experiencing.
2. Any medications you are currently taking.


On arrival for your appointment, Camille or Melissa
will review your paperwork, then take you for an initial
set of X-rays, (if warranted). You will then be escorted
to a comfortable examining area where you will meet
with Dr. Homer to discuss your concerns and plans for
a healthy and beautiful smile.

The initial consultation will be a relaxed one where
Dr. Homer will examine your current oral health, review
your x-rays, and make recommendations for future care and procedures. Whenever possible,
treatment will be administered on the same day.

Camille or Melissa will then be happy to review fees, insurance and other payment
options and help coordinate any scheduling necessary.

Insurance
Please be aware, unless other arrangements
have been made, we require full payment
after receiving treatment. Cherished
Smiles is happy to file your primary
insurance, but do advise that some of your
treatment may not be covered by your
insurance carrier. Treatment not covered by
your insurance, or any unpaid insurance
balance, is always the patients responsibility.
